JOB SUMMARY

Provides personal care and related services in the home, under the direction, instruction and supervision of the Services Supervisor.

Tasks to be performed by a Caregiver must be assigned by and performed under the supervision of a qualified supervisor who will be responsible for the services provided.

Under no circumstances may a Caregiver perform any intravenous procedures or any other sterile or invasive procedures, including rectal temperatures or enemas.

Caregivers may only provide medication assistance (reminders) if they have documented training from an RN.

POSITION RESPONSIBILITIES

1) Follows the plan of care to provide safe and competent care/service to the service recipient.

2) Helps the service recipient maintain good personal hygiene and a safe, healthy environment.

3) Plans and prepares nutritious meals.

4) Assists the service recipient with ambulation as approved.

5) Encourages independence according to the service plan.

6) Promotes mental alertness through activities of interest.

7) Provides emotional and psychological support and builds trust with the service recipient and family.

8) Reports any changes in the service recipient’s mental or physical condition to the supervisor.

9) Performs routine housekeeping tasks to maintain a safe and comfortable environment.

10) Prepares visit reports promptly and updates records weekly.

11) Confirms weekly scheduling of visits for coordination with other staff.

12) Works with personnel from other community agencies involved in care.

13) Assists with medications only if trained and authorized by an RN.

14) Communicates with the Supervisor regarding service recipient status and care planning.

15) Attends required in-service training sessions.

16) Promptly reports any changes in condition to the Supervisor.

17) Works within the Caregiver’s scope of practice under supervision.

JOB CONDITIONS

1) Must be able to drive and access homes that may not be wheelchair accessible.

2) Must have adequate hearing, eyesight, and physical dexterity for care duties.

3) May require bending, stooping, reaching, and moving up to 250 pounds occasionally.

4) May need to lift and/or carry up to 30 pounds.

5) Must communicate clearly both verbally and in writing.

EQUIPMENT OPERATION

1) Hand washing materials.

COMPANY INFORMATION

1) Has access to service recipient records and communicates with the supervisor as needed.

QUALIFICATIONS

1) Preferably a high school diploma or equivalent.

2) Must be able to understand, read, and write English.

3) Must be free from health problems that may be harmful to service recipients, self, or co-workers.

4) Must understand basics of personal care, housekeeping, and meal preparation.

5) Must respect service recipient rights, ethics, and confidentiality.
